Baby Kate Report (2003) This investigation examined the adequacy of the actions of the then Department of Families and Queensland Health in relation to the safety, wellbeing and care of baby Kate, who died at the age of 10 weeks. The report identified systemic problems within the then Department of Families and communication problems with Queensland Health regarding the management of child protection cases. The report recommended establishing a specialist body external to the department to review deaths of children who had contact with the department. The report was tabled on 6 October 2003.
